Below you will find important patient information you may need for the Northern Virginia Pulminary and Critical Care Assocites.

Making Appointments

  • We see all patients on an appointment basis and ask that you call in advance so that we may reserve time for you.
  • If you are unable to keep an appointment, 24-hour notice is appreciated so that this time may be given to another patient.
  • If you do not need to see a physician, but need to have blood drawn or an injection, please call the office to make an appointment.
  • If you are more than 15 minutes late for your appointment, we may not be able to see you. We will check with the doctor and may ask you to reschedule. Seeing patients who come later than the scheduled time causes the physician’s schedule to “back-up” causing all scheduled patients to see the doctor in an untimely manner. Every effort is made to keep wait time to a minimum

First Time Office Visits

If you are visiting one of our offices for the first time either as a new patient consultation or as a follow-up to hospital visits, please bring the following with you:

  • Your x-rays (actual films or CD, not just a report)
  • Your current insurance card
  • A list of your current medications, including dosages, with any refills you need, which the medical assistant will then attach directly to your chart
  • A referral, valid for one visit and spirometry, if your insurance requires it
  • Your driver’s license or a current photo i.d.
  • A copy of the results of any recent breathing tests
  • Completed patient forms (Patient health history & registration form)

Due to the nature of our practice, please be considerate and do not wear perfume, cologne, or other fragrances to our office, as they may worsen other patients’ conditions. Also, please treat our staff kindly and courteously as we are here to help you.

Emergency Care

  • In the event of a severe emergency, go immediately to the emergency room of the nearest hospital and ask them to contact your doctor. Our physicians have privileges at Inova Fairfax Hospital, Virginia Hospital Center – Arlington.
  • In all other emergency situations when the office is closed, telephone 703.641.8616 or 703.276.1916, press “1” on your touch-tone phone for the emergency line, leave a message including your phone number, and the physician on-call will return your call after you hang up. If you do not have a touch-tone phone, wait on the line for further instructions.
  • If an urgent situation arises while the office is open, call the office and the doctor will be in touch with you either directly or through office personnel giving you further instructions.

Telephone Calls

We encourage you to call with questions you may have regarding your health care. Our staff is trained to answer most questions; if you have questions that can only be answered by the doctor, please leave a message in our medical assistant’s voice mail.

Prescriptions and Renewals

  • Please renew your prescriptions by calling your pharmacist and giving him or her the refill number from your prescription bottle or packaging.
  • If you are requesting a new prescription, please speak to our medical assistants and give them the name of the medication you are requesting along with your pharmacy phone number. After approval from the physician, the prescription will be called in.
  • Please allow at least 24 hours (1 working day) for refill authorizations.

Patient Instructions

Preparation for methacholine challenge test

  1. No short-acting inhaled bronchodilators (Albuterol, Proventil, Ventolin, Xopenex, Pro-Air, etc.) for 8 hours prior to testing.
  2. No medium-acting inhaled bronchodilators (Atrovent) for 24 hours prior to testing.
  3. No long-acting inhaled bronchodilators (Advair, Serevent, Foradil, Spiriva, Brovans, etc.) for 48 hours prior to testing.
  4. No inhaled steroids (Aerobid, Azmacort, Advair, Asmanex, Flovent, Pulmicort, Symbicort, etc.) for 48 hours prior to testing.
  5. No antihistamines (Allegra, Atarax, Cetirizine, Clarinex, Claratin, Fexafenadine, Loratadine, Zyrtec, etc.) for 3 days prior to testing.
  6. No Theophylline, Uniphyl, or Theo-24 for 48 hours prior to testing.
  7. No Cromolyn Sodium (Intal), or Nedocromil (Tilade) for 8 hours prior to testing.
  8. No Leukotriene modifiers for 24 hours prior to testing.
  9. No caffeine the day of the test (coffee, tea, sodas, chocolate).
  10. NO SMOKING 24 hours prior to testing.
  11. DO NOT stop taking prescribed nasal sprays prior to the test.

If you do not see your inhaler listed here, please ask if you need to stop using it prior to taking this test.

Preparation for pulmonary function test (PFT)

  1. No short-acting inhaled bronchodilators (Albuterol, Proventil, Ventolin, Xopenex, Pro-Air, etc.) for 4 hours prior to testing.
  2. No Atrovent for 4 hours prior to testing.
  3. No long-acting inhaled bronchodilators (Advair, Serevent, Foradil, Spiriva, Brovans, etc.) for 12 hours prior to testing.
  4. No Theophylline, Uniphyl, or Theo-24 for 12 hours prior to testing.
  5. No caffeine the day of the test (coffee, tea, sodas, chocolate).
  6. NO SMOKING 24 hours prior to testing.

If you do not see your inhaler listed here, please ask if you need to stop using it prior to taking this test.

Insurance

Your health care insurance coverage may cover some, but not all, of the cost of your treatment. Most plans have copayments, deductibles, and other expenses which must be paid by the patient. Please bring your insurance card and copay with you to your visit. Please bring a current referral with you to your appointment, if one is required by your insurance company.

Even though we do participate with some insurance programs, it is important to remember that your insurance is a contract between you and your insurance company. As such, we do not file claims to commercial insurance carriers, except as a courtesy to our hospital patients. We will make every reasonable effort to assist you. If you have further questions, please refer to our payment policy or call our billing office at 703.641.8807.

We have contracted to participate with the following companies in their HMO and/or PPO products. Each physician’s participation with individual carriers is subject to change without notice. Patients should call their insurance carriers directly to verify their physician’s current participation status with their insurance plan. Our participation with these carriers is strictly as pulmonary and critical care specialists.

Payment Policy

If we do not participate with your insurance plan, payment in full is expected at the time services are rendered. VISA, Mastercard, Discover, personal checks, and cash are accepted. An itemized statement is available to you as you leave the office for you to file with your insurance carrier for reimbursement. If at any time payment poses a financial hardship, individual arrangements can be made and should be discussed with our billing staff prior to your appointment.

Payment for the deductibles, coinsurance, and copays for participating insurance plans is expected at the time of service. If you have an HMO that requires a referral, the referral must be brought with you at the time of the appointment or we will be unable to see you without payment in full.

Privacy Practice


Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully and complete the acknowledgement form.

Disability and Misc. Forms


Due to the time and attention it takes to complete Disability forms we will be charging a $25.00 fee for completion.  This charge is not billable to your insurance and must be paid when the forms are submitted to our office for completion.  You will be contacted when the forms are ready to be picked up, or we can mail them to the address on file in your patient record per your written request.
